Fee Schedule: $100 per ½ Hour
- Payment is due at the time of service.
- Methods of payment are Credit Card or Check. (You may choose to keep your credit card information online here for convenient secure automatic charging of future services.)
- All visits are timed from the time the appointment begins; you will only be billed for the actual time used.
- My services are Out-of-Network for all insurance plans, including Medicare. The reason is that insurance reimbursement is based on fees for standardized 15-to-20-minute Office Visits, and do not recognize a prolonged time-based Office Visit that we need for doing competent Functional Medicine care. (This is one reason why most other doctors do not practice this way and insurance companies are not familiar with it.)
(Some outside lab tests may be partially covered by some insurance plans, but patients are responsible for paying any lab-fee balances. I will discuss lab tests and costs before ordering.)
Cancellations: If you are unable to keep your scheduled appointment, please notify me at least 24 hours before your scheduled time.

- You are responsible for all charges for Office Visits and Lab Tests.
- Please contact me if you are not clear on any office policies or procedures.
- I can not manage medical emergencies. If you have a medical emergency, you must contact your primary care physician or dial 911!
